Once you have a signed Kyu Grading Authorisation (download here), please use this form to formally apply and pay for your grading examination.

The current cost for grading is $30 +$1 for card processing.

Please note that you will have to bring your Kyu Grading Authorisation on the day of your exam AND have submitted this form.
